The Enhanced Health in Care Homes (EHCH) aims to provide proactive care that is centred on the needs of individual residents, their families and care home staff.

We are supporting a number of  care homes in our locality. Our care home team are visiting the homes regularly and completing Individual Personalised Advanced Care Plans for the residents  and actively participating in weekly MDT meetings to support our residents locally.
